Dart Chart's fax data entry method is an easy to use paper form which the caregivers fill out by shading in bubbles. Paper work is kept to a minimum by having all three shifts document observed conditions on a single form.

For certain facilities reliable internet connectivity is a problem. If your facility is in an area without affordable high speed internet connectivity, or if the facility is unable to support a basic internal network, the paper based solution may provide a viable alternative to our WebForms product.

We encourage facilities to utilize this method only if basic technological infrastructure is a problem. Facilities utilizing WebForms on average notice an additional reimbursement jump due to 100% compliance with form completion due to workflow management and system enforcement of documentation rules.

Intake Referral Form: This form is utilized to ensure that the patient is Medicare qualified on the basis of their hospital stay.

Nursing Form: This is the standard daily documentation resource tool that is completed by your nursing staff for each tracked patient on a daily basis.

CNA Form: Utilizing our proprietary Talk Guide®, Touch Guide®, and Take The Weight Guide® approach, this form is completed by your CNA staff.
